The revised Release of Liability general form and Release of Liability form for leaving future medical liability open are available and are the exclusive forms for filers to use when filing a Release of Liability with the court. Additional language or changes of any kind to these forms will result in the filing being rejected by the court clerk. Additionally, no attachments other than a list of medical expenses incurred as a result of the alleged accident(s) and injury(s) shall be accepted with either Release of Liability form.
